The efficacy and application of humane nursing care in the care of patients undergoing kidney stone surgery

AbstractObjective:To explore the clinical effects of implementing humane nursing care after surgery in patients with kidney stones.Methods:a total of 110 patients with kidney stones treated surgically in our hospital from February 2021 to February 2022 were selected and pided into control and experimental groups(60 patients each)using a random number table method.The control group used a routine nursing intervention,and the observation group implemented humane nursing care on the basis of the control group.Clinical nursing outcomes,postoperative pain scores,sleep efficiency,nursing satisfaction,and psychological status during the intervention were compared between the two groups.Results:the total nursing effectiveness rate of 93.3%in patients after applying humane nursing care in the experimental group was significantly higher than that of 76.7%in the control group(P<0.05);Patients in the experimental group had significantly lower pain scores compared with the control group at different time points after surgery(P<0.05);Patients in the experimental group had a significantly higher sleep efficiency of(72.49±3.64)points compared with(60.26±3.62)points in the control group(P<0.05),their complication rate was 5.0%compared with 15.0%in the experimental group(P<0.05),and their satisfaction with care was 95.0%compared with 73.3%in the control group(P<0.05);After nursing intervention,the SAS and SDS scores of patients in both groups were improved,and the SAS and SDS scores of patients in the experimental group were significantly lower and significantly different from those of the control group(P<0.05).Conclusion:the adoption of humanities nursing intervention after the implementation of surgery for patients with kidney stones can improve the postoperative outcomes of patients,improve the negative emotions of patients,help the recovery of postoperative body function of patients,improve nursing satisfaction,improve anxiety,depression psychology,and be clinically sustainable for promotion.

Renal calculus is a common disease in the urinary system,with a clinical incidence of about 5%.The clinical symptoms of the patient are lumbago and abdominal pain,nausea,vomiting,pain in urine and even blood in urine.The clinical treatment of renal calculi is mostly surgical surgery,but with the rapid development of minimally invasive surgery technology,relying on its significant advantages,minimally invasive surgery for renal calculi has been widely promoted.Minimally invasive surgery will not cause too much stress reaction to patients,and patients are easy to accept[1].Complications of different degrees will occur after any operation,which is not conducive to the rehabilitation of patients.In addition,patients'sleep quality and psychological state are poor due to incision pain after surgery,and the significance of effective nursing services for patients undergoing kidney stone surgery is not only to improve the quality of life of patients,but also to improve nursing satisfaction[2-3].In this paper,120 patients with renal calculi treated by surgery were selected as the object to explore the application of humanistic nursing care in the nursing of patients with renal calculi.The report is as follows.

1 Data and methods

1.1 Clinical data

120 patients who underwent renal stone surgery in our hospital from February 2021 to February 2022 were selected as the study subjects.They were pided into two groups according to the random number table method,60 patients in the control group and 60 patients in the experimental group.There were 41 males and 19 females in the control group;The average age was(51.06±2.51)years,ranging from 25 to 74 years old;The duration of illness ranged from 6 months to 7 years,with an average duration of(5.19±0.35)years;Location of stones:48 cases of unilateral stones and 12 cases of bilateral stones.There were 42 male patients and 18 female patients in the experimental group;The average age was(51.12±2.48)years old,ranging from 24 to 75 years old;The duration of illness ranged from 6 months to 8 years,with an average duration of(5.12±0.37)years;Location of stones:47 cases of unilateral stones and 13 cases of bilateral stones.There was no significant difference in general clinical data between the two groups(P>0.05),which was comparable.

Inclusion criteria of subjects:(1)The age of patients≥18 years;(2)All patients were diagnosed as kidney stones by imaging examination;(3)No abnormality of renal function;(4)All patients voluntarily signed the informed consent form;(5)Good compliance;(6)No mental and language disorders;

Exclusion criteria:(1)patients with serious cardio-cerebrovascular diseases;(2)With heart,liver and kidney dysfunction;(3)Patients in pregnancy or lactation;(4)Suffering from coagulation dysfunction;(5)Have mental illness or cognitive impairment.

1.2 Intervention methods

In this study,all the interventions implemented by patients were jointly completed by the same medical staff.After the operation,the control group was given routine basic nursing intervention,such as basic clinical nursing,recovery tracking,medication guidance,diet and rest nursing,complication prevention nursing,etc.

The patients in the experimental group implemented humanistic care nursing intervention on the basis of routine nursing,and the specific contents are as follows:

(1)Ward environment nursing:create a warm and comfortable ward environment for patients,maintain proper ventilation in the ward,regularly disinfect the ward,do a good job of shading treatment,and reasonably control the temperature in the ward;Screens or curtains can be used to shield patients from personal privacy,and health publications,brochures,etc.can be placed in wards,corridors or nurse stations to improve patients'health knowledge level indirectly.(2)Psychological nursing:nursing staff should effectively communicate and solve problems with patients after surgery,and eliminate the anxiety and fear of patients due to lack of knowledge of kidney stone disease through psychological intervention;In addition,we will guide patients how to learn self-psychological guidance,such as listening to music,reading,deep breathing,and other ways;Emotional care,the support and care of relatives and friends to patients can increase the sense of belonging and kinship of patients,and further improve the treatment compliance of patients;Use the way of telling patients successful cases to improve patients'treatment self-confidence.(3)Sleep care:create a good sleep environment for patients.For example,patients with poor sleep quality can be uniformly pided into the same ward.When they are in the ward,they can amuse themselves by reading,listening to music and other ways.They can actively talk with others.The daytime sleep time should not be too long.It should be controlled within 1 hour to avoid modifying the sleep quality for a long time at night.The treatment process is usually completed in white,and the alarm should be set at a reasonable location;The medical staff should arrange the nursing staff to do night patrol,turn on all lights at night,and prevent the strong light from stimulating the patients to affect the rest;Night nursing should be completed in a centralized manner.When the patient is asleep,he/she should keep moving slowly and softly,and speak softly to prevent waking the patient.(4)Pain nursing:inform the patient of the postoperative pain in advance,and explain that the pain is a normal phenomenon,so that the patient can maintain an optimistic attitude,relax and cooperate with the treatment;In addition,we should also encourage patients to actively cooperate with the treatment and treat pain correctly.Patients can take a healthy lateral or supine position,with soft pillows clamped on both knees,to prevent increased pain caused by touching the fistula;Instruct patients to learn how to relieve pain during treatment.They can do breathing training and relaxation training.They can choose ways to accelerate blood circulation,such as appropriate ice compress,hot compress or local massage,so as to relieve patients'pain;Patients with severe pain should actively communicate with the doctor and take symptomatic medication under the doctor's instructions.

1.3 Evaluation criteria

(1)Compare the nursing effect of patients,and effectively judge the nursing effect.Remarkable results:high sleep quality,no complications found;Effective:good sleep quality with slight complications;Ineffective:poor sleep quality,serious complications and increased pain.Total nursing effect=obvious+effective.

(2)Observe the postoperative pain score,sleep efficiency score,complication rate and nursing satisfaction of the patients in the two groups.The higher the score of VAS(visual simulation score),the more severe the pain of the patients.The higher the score of sleep efficiency,the better the sleep treatment of the patients;The judgment standard of nursing satisfaction is based on the nursing satisfaction questionnaire of our hospital.

(3)The psychological state of the two groups of patients after operation can be determined by using the Self-Rating Anxiety Scale(SAS)and Self-Rating Depression Scale(SDS).

1.4 Statistical treatment

In this study,SPSS 20.0 software was used for statistical analysis and processing of the data.The count data in the clinical data were expressed by(±s),the statistical inference of the data was verified by chi-square(x2),the measurement data in the clinical data were expressed by(%),and the difference of the data samples was tested by t test.The results of the comparative test between the data showed that there was statistical difference(P<0.05).

2 Results

2.1 Comparison of nursing effects between two groups of patients

The total effective rate of nursing care of patients in the experimental group after humanistic care was 93.3%,and that of patients in the control group after routine basic nursing intervention was 76.7%.The difference between the two groups was significant(P<0.05),as shown in Table 1.

2.2 Comparison of pain scores at different time points after operation between the two groups

The pain score of patients in the experimental group at different time points after operation was significantly lower than that in the control group(P<0.05),as shown in Table 2.

2.3 Comparison of sleep efficiency score,complication rate and nursing satisfaction between the two groups

The sleep efficiency of the patients in the experimental group was(72.49±3.64)points higher than that in the control group(60.26±3.62)points,and the difference was significant(P<0.05).The complication rate of the patients in the experimental group was 5.0%lower than that in the control group(15.0%),and the difference was significant(P<0.05).The nursing satisfaction of the experimental group was 95.0%higher than that of the control group(73.3%),and the difference was significant(P<0.05),as shown in Table 3.

2.4 Comparison of psychological status between two groups of patients undergoing kidney stone surgery

After nursing intervention,the SAS and SDS scores of patients in the two groups were improved,and the SAS and SDS scores of patients in the experimental group were significantly lower than those in the control group(P<0.05),as shown in Table 4.

3 Discussion

The incidence rate of kidney stones is relatively high in clinical practice,which belongs to the common urinary system stone disease.The incidence of kidney stones tends to be young and middle-aged patients,and the clinical manifestations are vomiting,blood in urine,nausea and other symptoms.Once the patients with kidney stones have severe pain,they often use analgesics to control the pain during treatment.During the illness,patients'psychological state and mood will be affected to some extent,which is not conducive to the follow-up treatment work[4].With the continuous progress of medical technology,the application of minimally invasive technology has become more and more widespread,and the difficulty of treating kidney stones has also been reduced.It can not only reduce the degree of pain and surgical trauma of patients,but also there will be complications of varying degrees.Therefore,it is necessary to continue to deepen nursing work to obtain better clinical nursing level[5].

Humanistic nursing is a relatively advanced nursing method.It takes people as the concept of nursing,takes the will of patients as the goal of service,fully protects the interests of patients,fully pays attention to the psychological and physiological status of patients,and embodies the care of personnel in every aspect of work,so as to better and more scientifically implement nursing services to patients.Humanistic nursing care mainly focuses on ward care,psychological care,pain care,sleep care and other aspects.By providing patients with warm and comfortable ward environment,it can greatly improve the mood of patients,and can also effectively improve the functional function of the body[6].In addition,psychological nursing is mainly to actively communicate with patients,dredge their bad emotions and improve their treatment self-confidence.Pain nursing mainly reduces the pain of patients through relevant training and massage[7].Sleep nursing is based on patients'sleep habits,reducing ambient noise,improving sleep quality,promoting postoperative body,nursing intervention can improve patients'postoperative effects,improve patients'negative emotions,help patients recover their postoperative body functions,improve nursing satisfaction,improve anxiety and depression psychology,and promote clinical sustainable promotion

[8-9].

The results of this study showed that the total effective rate of nursing care for patients in the experimental group after humanistic care was 93.3%,while the total effective rate of nursing care for patients in the control group after routine basic nursing intervention was 76.7%;This shows that humanistic care can effectively improve the nursing effect of patients and help them recover.The pain score of patients in the experimental group was significantly lower than that in the control group at different time points after surgery,which showed that the implementation of humanistic nursing care measures for patients at different time points after surgery could reduce the pain of patients.The sleep efficiency of patients in the experimental group was(72.49±3.64)points higher than that in the control group(60.26±3.62)points,and the difference was significant(P<0.05).The complication rate of patients in the experimental group was 5.0%lower than that in the control group(15.0%),and the difference was significant(P<0.05).The nursing satisfaction of patients in the experimental group was 95.0%higher than that in the control group(73.3%),which showed that humanistic nursing care can effectively improve the sleep efficiency of patients Reduce complications and improve nursing satisfaction.After nursing intervention,the SAS and SDS scores of patients in the two groups were improved,and the SAS and SDS scores of patients in the experimental group were significantly lower than those in the control group,which showed that humanistic care could effectively improve the anxiety and depression of patients.

To sum up,humanistic care nursing intervention for patients with kidney stones after surgery can improve the postoperative effect of patients,improve their negative emotions,help patients recover their body functions after surgery,improve nursing satisfaction,improve anxiety and depression psychology,and promote clinical sustainable promotion.
