  • 简介:Aim:Toinvestigatetheeffectofhome-basedcardiacrehabilitationonfunctionalcapacity,healthbehavior,andriskfactorsinpatientswithacutecoronarysyndromeinChina.Methods:Eightypatientswithacutecoronarysyndromewereenrolledinthisprospectiverandomizedcontrolledstudy.Patientsinthecardiacrehabilitationgroup(n=52)receivedhome-basedcardiacrehabilitationwithaheartmanualandahomeexercisevideofor3monthsandpatientsinthecontrolgroup(n=28)receivedonlyroutinesecondaryprevention.The6-minwalkdistance,laboratorytestresults,healthybehavior(questionnaire),qualityoflife(12-itemShortFormHealthSurvey),anxiety(7-itemGeneralizedAnxietyDisorderQuestionnaire),anddepression(9-itemPatientHealthQuestionnaire)wereevaluatedatthebeginningandaftertreatmentfor3months.Results:Comparedwithbaselinedata,52patientswhoparticipatedincardiacrehabilitationhadlonger6-minwalkdistance(515.26±113.74mvs0.445.30±97.92m,P<0.0002),higherproportionsof“alwaysexercise”(78.26%vs.28%,P<0.05),“alwayslimitfoodwithsugar”(65.22%vs12%,P<0.05),“alwayseatfruits200–400geveryday”(82.61%vs.4%,P<0.05).and“alwayseatvegetables300–500geveryday”(21.74%vs.12%,P<0.06)aftertreatmentfor3months.Thelow-densitylipoproteincholesterolcontrolrate(52.17%vs.28%,P<0.05)andthesystolicbloodpressurecontrolrate(100%vs.68%,P<0.05)werealsosignificantlyincreasedaftertreatmentfor3monthsinthecardiacrehabilitationgroup.Nosignificantincreasewasfoundinthecontrolgroupaftertreatmentfor3months.Nocardiac-eventrelatedtohomeexercisewasreportedinbothgroups.Conclusion:Home-basedcardiacrehabilitationisafeasibleandavailablecardiacrehabilitationmodeinChina.

  • 简介:AbstractBackground:Deep brain stimulation (DBS) has proved effective for Parkinson’s disease (PD), but the identification of stimulation parameters relies on doctors’ subjective judgment on patient behavior.Methods:Five PD patients performed 10-meter walking tasks under different brain stimulation frequencies. During walking tests, a wearable functional near-infrared spectroscopy (fNIRS) system was used to measure the concentration change of oxygenated hemoglobin (ΔHbO2) in prefrontal cortex, parietal lobe and occipital lobe. Brain functional connectivity and global efficiency were calculated to quantify the brain activities.Results:We discovered that both the global and regional brain efficiency of all patients varied with stimulation parameters, and the DBS pattern enabling the highest brain efficiency was optimal for each patient, in accordance with the clinical assessments and DBS treatment decision made by the doctors.Conclusions:Task fNIRS assessments and brain functional connectivity analysis promise a quantified and objective solution for patient-specific optimization of DBS treatment.Trial registration:Name: Accurate treatment under the multidisciplinary cooperative diagnosis and treatment model of Parkinson’s disease.   • 简介:Heterogeneousnuclearribonucleoproteins(hnRNPs)arespliceosomalmacromolecularassemblagesandthusactivelyparticipateinpre-mRNAmetabolism.Theyarecomposedofevolutionarilyconservedandtandemlyrepeatedmotifs,wherebothRNA-bindingandprotein-proteinrecognitionoccurtoachievecellularactivities.Byyetunknownmechanisms,theseribonucleoprotein(RNP)particlesaretargetedbyautoantibodiesandhenceplaysignificantroleinavarietyofhumansystemicautoimmunediseases.Thisfeaturemakesthemimportantprognosticmarkersintermsofmolecularepidemiologyandpathogenesisofautoimmunity.SinceRNPdomainisoneofthemostconservedandwidespreadscaffolds,evolutionalysesoftheseRNA-bindingdomainscanprovidefurthercluesondisease-specificepitopeformation.ThestudypresentedhereinrepresentsasequencecomparisonofRNA-recognitionregionsofrecentlyclonedandcharacterizedhumanhnRNPA3withthoseofotherrelevanthnRNPA/B-typeproteins.Theirimplicationsinhumanautoimmunityareparticularlyemphasized.

  • 简介:Thecurrentstudydemonstratedthatinjuryofthespinalcordlateralfuniculusoccursinlivercirrhosis.Thisstudysoughttocomparethemorphologyofthethoracicandlumbarcord,theexpressionoffunctionalproteins,andchangesinvesselsbetweenlivercirrhosisandnon-cirrhosiscorpses.Resultsshowedthatinthelivercirrhosisgroup,thehepaticveinexpanded,thegastrointestinaltractwasfullofcoagulatedblood,blood-stasiswaseasilyseenintheveniplexofthevertebralcanalandthelumbarspinalcord,andthecellbodiesoftheanteriorhorninthethoracicandlumbarcordweresmallerthanthoseinnon-cirrhosiscorpses.Inaddition,nervecellsshrank,Nisslbodieswereconcentratedwithobscurednuclei,andneurofilamentandsynapsincontainingcellbodiesoftheanteriorhornandwhitematterdecreasedinthelivercirrhosisgroup.Theseexperimentalfindingsindicatethatabnormalcirculationofthespinalcord,resultingfromhemodynamicchangeofcirrhoticportalhypertension,maybethemostsignificantcauseofhepaticmyelopathy.

  • 简介:Acupunctureiswidelyusedtotreatfunctionaldyspepsiawithsatisfactoryoutcomes.CombinationoftheHeandMuacupointsiscommonlyusedandhasasynergisticeffectonfunctionaldyspepsia;however,itsunderlyingmechanismsremainunclear.Therefore,arandomizedcontrolledparallelclinicaltrialiscurrentlyunderwayatChengduUniversityofTraditionalChineseMedicine,China.ThistrialisdesignedtoexploretheefficacyofandcentralresponsestotheHe-Mupointcombinationinpatientswithfunctionaldyspepsiausingfunctionalmagneticresonanceimaging.Atotalof105patientswithfunctionaldyspepsiawillbeallocatedinto3groups:thelow-Hepointgroup(puncturingatZusanli(ST36)),Mupointgroup(puncturingatZhongwan(CV12)),andHe-Mupointcombinationgroup(puncturingatST36andCV12).Everyparticipantwillreceive20sessionsofmanualacupuncturefor4weeks.Theneedleswillbeinsertedperpendicularlytoadepthof1to2cun.Theangleofrotationandtwistingwillrangefrom90to180degrees,whileliftingandthrustingwillrangefrom0.3to0.5cm.Thevariousmanipulationswillbeperformed60to90timesperminute.Theneedleswillremaininplacefor30minutes,duringwhichmanipulationwillbeappliedevery10minutes.Magneticresonanceimagingwillbeperformedbeforeandafter20sessionsofacupuncture.TheprimaryoutcomeissymptomimprovementaccordingtotheChineseversionoftheNepeanDyspepsiaIndex.SecondaryoutcomesincludetheLeedsdyspepsiaquestionnaire,Self-RatingAnxietyScale,Self-RatingDepressionScale,BeckAnxietyInventory,BeckDepressionInventory,andvisualanaloguescalescoresbeforeandafter10and20sessionsofacupuncture.Needlesensationandadverseeventswillbeusedtoassessthetherapeuticeffects.ThisstudywillpromotemorewidespreadawarenessofthebenefitsofacupointcombinationintheclinicalsettingandprovideafurtherexplanationoftheneuromechanismbywhichacupunctureattheHe-Mupointcombinationforfunctio

  • 简介:Refractorymaterialsaresubjectedtomechanicalandchemicalloadsduringapplication,whichrespectivelyleadtofractureandcorrosion.Theunderstandingofthemechanicalfractureandthemicrostructuralmodificationafterprogressivethermalshocksisnecessaryforthesuitabledesignofhighperformancerefractorymaterials.Thefractureprocesszonecanbedividedintotwoareas,wheredifferenttougheningmechanismsarelikelytotakeplace:thecracktipandthefollowingprocessregioncalledwakeregion.Inthisstudy,theemphasisislaidonthemicrostructuralmechanismsoccurringinthewakeregionofthecrackandthecracktip.Therefore,functionalaggregates,namelyfusedeutecticaggregatesofAl2O3-ZrO2-SiO2andAl2O3-ZrO2,andandalusite,areaddedtoamodelhighaluminacastableformulationbasedontabularalumina.Thediscrepancyinthermalbehaviorbetweenaggregatesandmatrixaswellasphasetransformationsuchasmulliteformationfavorfurtherthenucleationofmicrocracksthatreducethestressfieldatthelevelofthecracktip.Twodifferentgrainfractionsoftabularalumina(0.2-0.6mmor2.24-3.00mm)aresubstitutedbythesefunctionalaggregates.Aftersinteringat1500℃for6h,thesamplesarequenchedupto10timesinairwithoutpressurefrom950℃toroomtemperature.Theresultingelasticpropertiesareexaminedaftereachthermalshockcycle,aswellastheresidualbendingstrengthafter0,1,3,5,7and10thermalshocks.TheseresultsarecorrelatedwithScanningElectronMicroscopyanalysesafterthedifferentthermalshockcycles.

  • 简介:AbstractBackground:Resting-state functional magnetic resonance imaging (rs-fMRI) is a promising method for the study of brain function. Typically, rs-fMRI is performed on anesthetized animals. Although different functional connectivity (FC) in various anesthetics on whole brain have been studied, few studies have focused on different FC in the aged brain. Here, we measured FC under three commonly used anesthesia methods and analyzed data to determine if the FC in whole brain analysis were similar among groups.Methods:Twenty-four male aged Wistar rats were randomly divided into three groups (n = 8 in each group). Anesthesia was performed under either isoflurane (ISO), combined ISO + dexmedetomidine (DEX) or α-chloralose (AC) according to the groups. Data of rs-fMRI was analyzed by FC in a voxel-wise way. Differences in the FC maps between the groups were analyzed by one-way analysis of variance and post hoc two-sample t tests.Results:Compared with ISO + DEX anesthesia, ISO anesthesia caused increased FC in posterior brain and decreased FC in the middle brain of the aged rat. AC anesthesia caused global suppression as no increase in FC was observed.Conclusion:ISO could be used as a substitute for ISO + DEX in rat default mode network studies if the left temporal association cortex is not considered important.

  • 简介:AbstractBackground:Follistatin-like 1 (FSTL1) plays both pro-inflammatory and anti-inflammatory roles in the inflammatory processes. We investigated whether serum FSTL1 could predict the current anti-neutrophil cytoplasmic antibody-associated vasculitis (AAV)-specific indices.Methods:We randomly selected 74 patients with AAV from a prospective and observational cohort of Korean patients with AAV. Clinical and laboratory data and AAV-specific indices were recorded. FSTL1 concentration was determined using the stored sera. The lowest tertile of the short-form 36-item health survey (SF-36) was defined as the current low SF-36. The cutoffs of serum FSTL1 for the current low SF-36 physical component summary (PCS) and SF-36 mental component summary (MCS) were extrapolated by the receiver operator characteristic curve.Results:The median age was 62.5 years (55.4% were women). Serum FSTL1 was significantly correlated with SF-36 PCS (r = -0.374), SF-36 MCS (r = -0.377), and C-reactive protein (CRP) (r = 0.307), but not with Birmingham vasculitis activity score (BVAS). In the multivariable linear regression analyses, BVAS, CRP, and serum FSTL1 were independently associated with the current SF-36 PCS (β = -0.255, β = -0.430, and β = -0.266, respectively) and the current SF-36 MCS (β = -0.234, β=-0.229, and β= -0.296, respectively). Patients with serum FSTL1 ≥779.8 pg/mL and those with serum FSTL1 ≥841.6 pg/mL exhibited a significantly higher risk of having the current low SF-36 PCS and SF-36 MCS than those without (relative risk 7.583 and 6.200, respectively).Conclusion:Serum FSTL1 could predict the current functional status in AAV patients.
