摘要
摘要目的研究超纯透析对维持性血液透析(Maintenancehemodialysis,MHD)患者贫血及氧化应激反应(OSR)的影响.方法选取我院２０１３年１１月至２０１４年１１月期间收治的接受MHD治疗患者１０２例,作为本次研究对象.根据数字法随机将其分为观察组和对照组,对照组患者接受普通透析方式治疗,观察组实施超纯透析.对比两组患者治疗前后贫血情况,两组患者治疗前后OSR相关指标变化情况,以及两组不良反应情况.结果治疗后两组的Hb及HCT水平均上升,β２－MG水平均下降,但观察组的的Hb及HCT水平显著高于对照组,而β２－MG水平显著低于对照组,差异均有统计学意义(均P＜０．０５).治疗后观察组的Alb水平显著高于对照组,MPO、CRP及MDA水平均分别显著低于对照组,差异均有统计学意义(均P＜０．０５).观察组的总不良反应率为７．８４％,与对照组的５．８８％相比,差异无统计学意义(P＞０．０５).结论超纯透析对于MHD患者的贫血及氧化应激状态具有较好的改善作用,效果明显,值得在临床上推广应用.关键词超纯透析;维持性血液透析;贫血;氧化应激反应;影响InfluenceofultrapuredialysisonmaintenancehemodialysispatientswithanemiaandoxidativestressreactionAbstractObjectiveTostudyinfluenceofultrapuredialysisonmaintenancehemodialysispatientswithanemiaandoxidativestressreaction．Methods１０２casesofpatientstreatedinourhospitalfromNovember２０１３toNovember２０１４byMHDastheresearchobject．Accordingtodigitalmethodrandomlydividedintoobservationgroupandcontrolgroup,Thecontrolgrouppatientsweretreatedwithregulardialysismode,theobservationgroupimplementationofultrapuredialysis,comparedthetwogroupsofpatientsbeforeandaftertreatmentofanemia,thechangesofOSRindexbeforeandaftertreatmentintwogroups,andtheadversereactionsofthetwogroups．ResultsHbandHCTlevelsinthetwogroupswereincreasedaftertreatment,β２－MGleveldecreased,butHbandHCTlevelsoftheobservationgroupwassignificantlyhigherthanthatincontrolgroup,whilethelevelofβ２－MGwassignificantlylowerthanthecontrolgroup,thedifferenceswerestatisticallysignificant(allP＜０．０５)．TheAlbleveloftheobservationgroupaftertreatmentwassignificantlyhigherthanthatincontrolgroup,MPO,CRPandMDAlevelsweresignificantlylowerthanthoseinthecontrolgroup,thedifferenceswerestatisticallysignificant(allP＜０．０５)．Thetotaladversereactionratewas７．８４％intheobservationgroup,comparedwiththecontrolgroup５．８８％,thedifferencewasnotstatisticallysignificant(P＞０．０５)．ConclusionTheultrapuredialysiscanimprovetheeffect,goodfortheMHDpatientswithanemiaandoxidativestresseffectisobvious,itisworthyofpopularizationandapＧplicationinclinic．KeywordsUltrapuredialysis;Hemodialysis;Anemia;Oxidativestress;Influence中图分类号R５５６文献标识码B文章编号１００８－６３１５(２０１５)１０－０３６１－０２
